This book follows the mischievous activities of fourth graders, George and Harold, as they pull numerous pranks at their school. They write a comic book about Captain Underpants and pull a series of practical jokes on the school's football team. Their principal, the mean Mr. Krupp, catches their pranks on video and blackmails the boys into doing work for him, morning, day, and night. They eventually get a Hypno-Ring which they use to hypnotize Mr Krupp into becoming Captain Underpants, who goes out into the city to stop crime. The three then come into contact with the evil Dr. Diaper, but they foil his plan to take over the world. When they return to the school George and Harold realize that they have misplaced the manual for the Hypno-Ring so they decide to dump water on Mr. Krupp's head to see if that will stop the hypnosis. It works, but what they don't know is that the manual states that duping water on the person's head will only work until someone snaps their fingers. So from then on, whenever someone snaps their fingers, Mr. Krupp turns into Captain Underpants.

This book has been repeatedly challenged due to its toilet humour, disrespect for authority, and concerns that it would cause unruly behaviour amongst school age children.
